It works like a big list of things that the doctors and nurses can look for. They might look at how much the person eats or drinks, how much they move around, and how they feel about their body. By looking at all of these different things, the doctors and nurses can get a better idea of how the person is doing and if they might need some extra help to start eating more and feeling better. It's kind of like when you go to the doctor and they ask you questions to make sure you're healthy!
