Okay kiddo, so imagine you're playing with a toy ball and it accidentally rolls onto a bunch of rocks. You know that if you try to get the ball back, you might hurt yourself because the rocks are sharp and pointy. But you also know that if you don't get the ball back, you'll feel sad.
Now imagine a soldier is on a battlefield and a bad guy throws a grenade near their friends. The grenade is like a toy ball, except it can explode and hurt everyone around it. The soldier knows that if they don't do anything, their friends might get hurt or even die. But they also know that if they try to throw the grenade away, it might not go far enough and their friends will still be hurt.
So what the soldier might do is jump on top of the grenade. Like putting their body over the toy ball to protect it, the soldier puts their body over the grenade to protect their friends. This would hurt the soldier a lot, but it might save the lives of their friends.
This kind of bravery is really important and soldiers who do this are very brave. It's called "falling on a grenade" because the soldier literally falls on top of the grenade to try to stop it from exploding and hurting anyone else.
