Herbert William Heinrich was an American scientist and safety consultant in the early 1900s. He was known for his work on safety in the workplace. He developed an approach to thinking about safety called the "Heinrich Triangle" which states that for every one accidental injury, there are 29 near-misses or close calls and 300 unsafe conditions or behaviors. He believed that if people focused on preventing those 300 unsafe conditions and behaviors, then many of the near-misses and accidents would be prevented. Basically, he wanted people to pay attention to the small things that might cause an accident and to make sure they were fixed before an accident occurred.
