It's when you start expecting the worst from people and think they're always trying to do something bad to you, even if it's not true. Sometimes it can be hard to tell if it's real or not, but it's important to try not to get too worried about it.
It's like when you're playing hide and seek and you think your friend is hiding and trying to trick you - but really, they might just be hiding to have fun and play the game.
So paranoid social cognition is when you feel like someone isn't being nice to you, even if they really are. It's important to try and understand that not everyone is trying to hurt you and to talk to someone you trust if you're feeling anxious or worried about it.
