Personally Controlled Electronic Health Record

A personally controlled electronic health record (also called a "PCEHR") is like an online filing cabinet for all of your health records. It's a place where you can put information about your health, like your allergies or your medicines, that can be shared with your doctors and other healthcare professionals. Your PCEHR is like your own personal record that you can use to keep track of your health. You can add information to your PCEHR anytime, like if you get a new medicine or if you visit a new doctor. This way, all of your healthcare providers have the most up-to-date information about your health.
