Enhanced Primary Care Case Management Program

An enhanced primary care case management program is a way for a doctor to coordinate the medical care for a particular patient. When someone enrolls in a program like this, the doctor (or case manager) will create a plan for that person's care. The plan includes things like the types of treatments needed and the order in which they should happen. The doctor also contacts other healthcare providers (like specialists and therapists) to make sure they are on the same page and everyone is giving the patient the best care possible.
