Origin and cause investigation is like trying to solve a mystery, just like a detective does in a storybook. Imagine that there is something wrong that happened, like a fire in a house or a car accident. An origin and cause investigation is when grown-up detectives (called investigators) come to the place to find out what happened and why it happened. They use a lot of tools and skills to solve this mystery, just like how you would try to solve a puzzle.
Firstly, the investigators need to find out the place where the problem started. They will look at all the clues like burnt marks or broken things, and try to figure out which part of the house or car was where the fire or accident happened. This is called the origin of the problem.
Next, the investigators will try to understand why it happened. This means they need to find out what caused the problem in the first place. They might analyze how things were arranged in the house or what happened during the car ride that led to the accident. This is called the cause of the problem.
Once the investigators have found out the origin and the cause of the problem, they can begin to make a plan to prevent it from happening again. So, just like how you would learn from your mistakes, grown-up detectives also learn from their investigation, and use the information they found to make things better for the future.
