Patient record access

When you visit the doctor, they write down things about you like your name, age, weight, height, and how you are feeling. They also write down what they did to help you feel better, like giving you medicine or checking your temperature.

All of these things they write down are called your "patient record." It's like a notebook that has all the information about your visits to the doctor.

Sometimes, you might need to go to a different doctor or a hospital, and they might need to see your patient record to know how to help you. But not everyone has permission to read your patient record. Only doctors and nurses who are helping you and your family can look at it.

So, to keep your patient record safe, doctors and hospitals have special rules about who can see it. They use special computer programs to keep track of who has looked at your patient record and why. This helps keep all your information private and protected.

If you want to see your own patient record, you can ask your doctor to show it to you. But you can't take it home with you because your patient record belongs to the doctor or hospital that wrote it.
