Community-based care

Community-based care means that people who need help with their health or other needs can get it from people in their own neighborhoods or communities instead of going to a hospital or big clinic far away.

In the same way, community-based care helps people get the help they need from people in their own community who know them well and can help them better because they understand their unique needs.

This kind of care can happen in different places, like a small clinic or even someone's home. There are different kinds of health workers who can provide this care, like doctors, nurses or social workers.

Community-based care is important because it can help people stay healthy and also save them time and money by not having to travel far away to get help.
