Continuity of care record

A continuity of care record (CCR) is like a file or folder that contains all of the important information about your health. It has information about doctors you have seen, hospital visits, lab results, medications, and any other medical care you've received. All of this information is gathered into one place, so that your doctor can easily look at it and understand your medical history. It also helps when you switch doctors; the new doctor can quickly get all your information and know right away what treatments you have already had and what you need.
