Medical record

Medical records are like a person's medical history. They are notes from doctors and nurses about what happens to a person whenever they go to the doctor. They tell doctors like when the person last got sick, if the person had any treatments or medicines, or if the person has any allergies. They also have information about test results, hospital visits and any surgeries a person may have had. Medical records help doctors know what to do to help the person if they get sick.
